• Players offered roster spots after tryouts will be asked to complete the 08/09 Registration form.

If you have been extended an invitation for the Summer options, please print this registration form and submit with $150 payment.         

• You can either drop off or mail the completed registration form and deposit to the WAA office ---
  ATTN: MYRA, 2323 Liberty Street, Aurora, IL 60502.
   
• If you prefer to fax the commitment form to (630) 978-2954, you will need to call with a MC or VISA (add a $4 convenience fee) to make your deposit.
   
• A $200 NON-refundable deposit will be required to secure your position on the roster. (Payment accepted by check, cash, Mastercard or Visa - add $4 for credit)
   
• This information is due as soon as possible to secure your spot.  Please get registered at your earliest convenience.
   

• When your deposit and form are turned in, you will receive a quick facts sheet.  A parent/player handbook will be distributed to each player at the beginning of the season.
   

• Uniforms will be sized and ordered in the summer.  You will be notified by email when it is time to get fitted.  Payment will be due when the order is placed.
